I will cooperate with the staff in doing temperature measurement, checking the national 14-day travel code and health code, and assisting the implementation of other related epidemic prevention and control measures.

2. I am not in the 14-day centralized isolation observation period or home isolation observation period.

3. I declare that I am in good health. I have never been diagnosed as a confirmed case of COVID-19 or an asymptomatically infected person; Or I was previously diagnosed as a confirmed case of COVID-19 or asymptomatic infection but have been cured and discharged from hospital and am not included in the follow-up medical observation period.

4. In the past 14 days before the festivals, I have not had a body temperature ≥37.3℃, and I did not have symptoms such as cough, sputum, sore throat, sneezing, runny nose, nasal congestion, headache, muscle pain, joint pain, shortness of breath, dyspnea, chest tightness, conjunctival congestion, abdominal pain, rash, jaundice and other symptoms. My personal health condition is normal.

5. If I have symptoms such as cough, fever and other physical conditions during the event, I will voluntarily cooperate with the staff to carry out relevant epidemic prevention and control measures.

6. I will abide by the relevant laws of the People's Republic of China and the relevant provisions of Shanghai Municipality on the prevention and control of infectious diseases, as well as the epidemic prevention measures of the festival committee. If I do not abide by the above commitments, I am willing to bear the corresponding legal responsibility.

If I make false promises such as concealing my medical history and contact history, intentionally containing my symptoms, concealing my health status or evading epidemic prevention measures, I am willing to bear the corresponding legal responsibility.
